Health Services Social Worker

Job overview

The Health Services Social Worker role is onsite in Black Mountain, NC, with compensation of $65,000–$68,000 per year. Givens Communities’ mission is to expand the possibilities of aging by improving lives through communities, services, and outreach. Serving the Health Care Center and Residential Assisted Living, this social worker provides navigation and support that contribute to individualized resident care. The role coordinates with care teams, residents, families, and outside agencies to support care planning, resident rights, admissions, and transitions.

What you'll do

• Serve as the social services point of contact for the Health Care Center and Residential Assisted Living
• collaborate with health services, nursing, and the interdisciplinary Care Plan Team
• complete psychosocial assessments and develop, implement, and monitor individualized care plans
• advocate for residents’ rights and follow up on complaints or grievances
• coordinate discharge planning and care transitions
• assist with admissions and provide backup to the admissions coordinator as needed.
