The Care Coordinator, Substance Use Disorder role is onsite in Chicago, IL. Pay is $19.61–$24 per hour. The coordinator works with the medication-assisted treatment (MAT) team at Primecare Community Health to support treatment engagement and care coordination.

What you'll do

• Conduct proactive patient outreach, coordinate care with the MAT primary care provider and behavioral health providers, track follow-up and outcomes, document encounters, facilitate clinically indicated referrals, support recovery plan changes in consultation with providers, and help patients access social services.
