What you'll do

• Travel to members’ homes and community settings to conduct required physical, cognitive, and functional assessments
• document findings in GuidingCare and send SOAP notes to the assigned Care Manager
• submit complete documentation within 48 hours
• resolve audit or review findings within two business days
• follow field-safety guidance, escalate clinical or safety concerns, comply with mandated reporting laws, HIPAA, and NCQA standards, and complete required training.
