Social Worker

What you'll do

• Provide social work services in accordance with licensure standards and applicable requirements
• complete individual, family, and group assessments within required timelines
• provide short-term crisis intervention as needed
• connect patients and families with community resources for post-hospital needs
• monitor progress toward discharge and completion of requirements
• assist with applications for assistance services such as Medicaid
• facilitate or assist with psychotherapy groups
• provide individual counseling and maintain required documentation
• attend interdisciplinary treatment team meetings.
