Speech-Language Pathologist, Skilled Nursing Facility

The Speech-Language Pathologist role is onsite at an in-house skilled nursing facility in Hopkins, MN; compensation is $50–$55 per hour. Evolve Therapy Services is a therapy management organization hiring for a partner facility. The pathologist plans and provides speech-language therapy to support resident care, evaluates patients, develops treatment goals, and instructs patients, families, and staff in procedures that support therapeutic carryover. The role also maintains compliant documentation and supports facility screening and swallowing-study protocols.

What you'll do

• Provide speech therapy and initiate referrals when appropriate
• evaluate patients and gather treatment-planning data
• develop treatment plans with short- and long-term goals
• instruct patients, families, and staff in appropriate procedures
• complete medically necessary documentation in NetHealth
• oversee and implement FEES/MBS protocols in coordination with site programs
• participate in resident screening protocols.
