What you'll do

• Provide individual and group counseling and ongoing assistance with social issues
• contribute social, emotional, cognitive, and cultural perspectives to individualized integrated care plans
• connect participants with community and financial resources
• support continuity of care
• conduct assessments and interventions in other care settings, including participants’ homes, as needed.
