The Bilingual Social Worker — Community Outreach role is onsite in Beachwood, NJ, with outreach across Monmouth and Ocean Counties. The schedule is Monday–Friday, 8:30 am–4:30 pm, with some nights and weekends possibly needed. Pay is $22 per hour. The role connects underserved women and families with health, social, and community services. The social worker conducts outreach, home visits, case management, education, and supportive services, with a focus on high-need women and maternal and child health. The role helps participants access care and resources, supports health education and engagement, and documents services and program outcomes.

What you'll do

• Conduct community outreach, education, home visits, case management, group activities, workshops, and supportive services
• focus outreach on high-need women not engaged with health or community services
• recruit and retain prenatal and inter-conception clients and families
• collect participant data and coordinate referrals
• assist with health insurance and Medicaid enrollment
• provide follow-up and needs assessment
• conduct monthly community education events
• help coordinate quarterly consumer advisory board meetings
• document activities and submit monthly data
• help clients access health services and overcome barriers
• build relationships with participants, providers, agencies, and community partners.
