On-Call Registered Nurse (RN) — Home Health & Hospice

The On-Call Registered Nurse (RN) — Home Health & Hospice role is an onsite field position serving Salt Lake County, UT. What you'll do

• Respond to after-hours patient and caregiver calls
• assess needs by phone and determine when an in-person visit is appropriate
• complete home health and hospice visits as needed
• assess changes in condition
• provide symptom management and ordered treatments
• educate and support patients, families, and caregivers
• communicate with the clinical team and providers
• document care accurately.
