The Licensed Practical Nurse (LPN) provides professional nursing care for residents of all ages in long term care, under the supervision of a registered nurse, advanced practice provider, or physician, demonstrating knowledge of nursing principles and age-appropriate care.

What you'll do

• Provide professional nursing care for residents of all ages in long term care, under the supervision of a registered nurse, advanced practice provider, or physician.
• Demonstrate knowledge of nursing principles for the resident.
• Demonstrate competency and practice within the full scope of nursing expertise/knowledge.
• Utilize appropriate age and population specific standards related to the physical and psychosocial needs of the resident as per care plan.
• Perform testing to evaluate data reflective of the resident's status and to provide the care needed.
• Recognize problems, recommend solutions and respond effectively to assist in an emergent situation.
• Ensure the health, comfort and safety of residents.
• Contribute to the assessment of residents and administer medications or treatments as ordered.
• Provide technical support to healthcare professionals.
• Document resident interactions and outcomes.
• Establish and maintain effective working relationships.
• Work effectively with professional and supportive personnel.
• Counsel residents, family members and/or resident advocate on prevention and treatment plans.
• Develop a self-care program.
• Perform other related duties as assigned.

What we're looking for

Education

Graduate from an accredited practical nursing program.

Experience

Graduate from an accredited practical nursing program. Associate degree in practical nursing is preferred. Must have attended a pre-licensure nursing program that included supervised clinical experience across the life span or have had a minimum of 400 hours of experience as LPN. Currently holds an unencumbered Licensed Practical Nurse (LPN) license. Obtains and subsequently maintains required department specific competencies and certifications.
