What you'll do

• Assess patients and establish and evaluate treatment programs, plans of care, goals, and discharge plans in collaboration with physicians and patients or families
• implement treatment plans and select activities to progress patients toward goals
• revise plans with patient and family input
• provide education and training
• give discharge instructions and referrals as appropriate
• document evaluations and treatment sessions
• participate in required education and professional development.
