RN Case Manager, Acute Rehabilitation

Manage rehabilitative care to ensure optimum outcomes; provide, coordinate, and direct care specific to the needs of each Rehab patient; collaborate with the Rehab Program Director and Facility Case Management Director; provide program orientation to patients/families/caregivers; coordinate patient/family/caregiver education; promote participation in team discussions.
