Family Case Management Case Associate

Job overview

The Family Case Management Case Associate role is onsite in Chicago, IL, with pay of $21–$24 per hour. World Relief is a Christian humanitarian organization whose programs include refugee and immigrant services and advocacy. The associate helps refugees and immigrants without Medicaid navigate healthcare resources and supports group activities that promote health knowledge and social connection. The role coordinates with World Relief staff, community organizations, local churches, and healthcare providers to help participants access services and maintain accurate case records.

What you'll do

• Provide safe transportation for group activities and outings
• help clients identify and access healthcare resources
• research providers and schedule appointments
• support participants with prescription management and medical paperwork
• assist with benefits applications and referrals
• connect participants to English classes and services
• maintain confidential case files
• support group activities and collaborate with staff, partners, and churches.

Why this role
