Case Manager

The Case Manager role is onsite in Leavenworth, WA, with an anticipated 40-hour Monday–Friday schedule and occasional weekend shifts; exact hours will be determined with the director. The Case Manager coordinates care and discharge planning, identifies barriers early, and supports appropriate admissions and transitions. Through collaboration with patients, families, providers, nursing, rehabilitation, social work, registration, and community partners, the role helps ensure safe, timely, well-coordinated care and continuity of services after discharge.
