What you'll do

• Participate in on-call duties per policy
• support quality management
• arrange equipment, supplies, and services
• review patient care and documentation
• complete initial OASIS assessments
• develop and update nursing diagnoses and care plans with patient and family input
• provide prescribed nursing procedures, medications, and treatments
• supervise LPNs and home health aides
• reassess patients after condition changes or hospital discharge
• plan discharges
• maintain clinical notes and update physicians
• educate patients and caregivers on self-care, medications, diet, and prevention.
