What you'll do

• Assess patient and family learning needs
• establish goals and develop individualized plans of care
• coordinate care and discharge planning with patients, families, providers, and community resources
• follow physician orders
• reconcile medications
• manage an assigned caseload
• triage visits, messages, and calls by urgency
• supervise home health aides
• document care accurately and promptly
• participate in quality, safety, staff meeting, and in-service activities
• perform on-call responsibilities and services for assigned patients and families.
