The nurse supports appropriate hospice admissions by gathering information, reviewing records, educating patients and families, and completing admission documentation. The role also builds referral relationships and coordinates with admissions and patient-care colleagues to support a responsive transition into hospice care.

What you'll do

• Participate in information gathering, analysis, education, and form completion during admissions
• evaluate patients for hospice appropriateness, including medical record review as requested
• build referral-source relationships
• maintain clinical and professional competence
• develop relationships with admissions and patient-care resources
• educate patients and families about hospice and palliative care and address their questions.
