What you'll do

• Provide hospice care focused on pain and symptom management
• respond to urgent calls, assess situations, and provide appropriate interventions
• administer and adjust medications while monitoring effectiveness
• collaborate with the interdisciplinary care team
• support and educate families
• document care, assessments, and interventions
• coordinate with the on-call team when additional support is needed
• participate in quality improvement and follow hospice protocols.
