What you'll do

• Provide intensive care coordination and case management
• facilitate Child and Family Team meetings
• conduct assessments and develop individualized care plans
• coordinate behavioral health, school, DCYF, juvenile justice, and other services
• collaborate with Youth and Family Peer Specialists
• maintain documentation and track progress
• support youth and families through WISe services.
