Case Care Specialist, Bexar, Hybrid

What you'll do

• Coordinate admissions, case planning, extensions, and discharges
• work with provider and childcare staff and community partners to deliver planned services
• make frequent post-placement contacts
• document services and maintain client files
• attend case meetings
• maintain relationships with DFPS and network providers
• support intake and placement, including transportation
• assess referred children’s strengths and needs
• and communicate with children, families, providers, and legal or judicial partners.

Why this role
