What you'll do
- Complete resident assessments on required schedules and when conditions change
- document and communicate resident changes
- complete MDS assessments, Care Area Assessments, and care plans
- coordinate with interdisciplinary departments
- maintain assessment schedules and transmission records
- submit MDS transmissions to the state
- monitor compliance with Medicare, Medicaid, and CMS guidelines
- assist with surveys, audits, quality measures, and reimbursement initiatives.
