What you'll do

• Conduct intake assessments, biopsychosocial evaluations, and substance use screenings
• develop and update treatment plans
• facilitate individual, group, and family counseling
• provide crisis intervention, relapse prevention, and recovery support
• educate patients and families
• document services in EHR
• collaborate with treatment providers
• participate in team meetings, case reviews, and discharge planning
• maintain confidentiality and professional boundaries.

Why this role
