Collaborative Care Case Manager

The Collaborative Care Case Manager role is onsite in Derry Village, NH. Pay is $26 per hour. Center for Life Management provides mental health services, and its Collaborative Care Program integrates mental, physical, and behavioral health support. The case manager develops treatment and care plans, coordinates benefits and community resources, and provides community-based counseling and support. The role helps clients with co-occurring mental health needs and intellectual disabilities or traumatic brain injury work toward improved functioning and wellness goals.

What you'll do

• Assess clients and develop treatment and care plans
• assist with medical coverage
• coordinate outreach, advocacy, resources, and services
• provide counseling and Functional Support Services in home or community settings as appropriate
• provide therapeutic behavioral services and family training as needed
• provide crisis intervention when necessary
• coordinate with treatment teams and external providers
• document services
• participate in consultation meetings and committees.

Why this role

Full-time, 40 hours per week. Eligible staff may apply for a tuition-covered Master of Science in Clinical Mental Health Counseling program, subject to eligibility and a five-year employment commitment.
