Role and Responsibilities. Responsible for developing and maintaining client relations and intake of direct referrals made to HCR. Cultivate relationships with our physicians and hospitals to facilitate the best Home Care solutions for their patients. Monitor HCR RightFax folders, label faxes and attach electronic documents to patient record in Home Care Home Base (HCHB).
