Preferred Communities Case Manager

What you'll do

• Assess client needs and eligibility
• maintain regular contact and conduct home visits as needed
• connect clients with public benefits and community services
• coordinate with staff and providers
• support appointment attendance and independent transportation
• provide cultural orientation and arrange interpretation or translation as required
• maintain case files, notes, and program database
• close and transfer cases
• conduct outreach and reporting
• recruit, orient, and supervise interns from Boise State University’s School of Social Work.
