What you'll do

• Coordinate case management and patient services
• educate and support patients, families, employees, and medical staff on case management
• collaborate with admissions and financial staff on pre-certification and admission planning
• review pre-admission screening for utilization, funding, and discharge issues
• begin discharge planning at admission
• assess discharge needs with the interdisciplinary team and refer to community care agencies as appropriate
• maintain professional relationships with healthcare facilities and organizations
• travel between the Pontiac and Garden City facilities.
