What you'll do

• Use individualized, multidisciplinary approaches to support patients

• support goals for independent functioning and rehabilitation
• use evidence-based interventions to develop social, family, community, and independent-living skills
• educate patients about substance use disorders and prescribed medications
• provide services in homes and community settings
• contribute to treatment plans
• assess progress and barriers
• document services
• identify relapse risks and develop recovery-support strategies.
