What you'll do

• Provide comprehensive nursing care by assessing, planning, implementing, and evaluating individualized care plans

• maintain timely patient documentation
• educate and support patients and caregivers
• provide support at the time of death
• participate in 24/7 care coverage alongside the hospice team
• attend team meetings and assist with hospice aide supervision.
