What you'll do

• Provide home health visits
• complete initial comprehensive assessments and documentation
• obtain and document physician orders
• initiate and reevaluate care plans with the Clinical Team Manager
• coordinate with physicians and the home care team
• complete recertifications, resumptions of care, transfers, and discharges as requested
• assess need for other team services
• conduct Home Health Aide supervisory visits
• follow regulatory requirements.

Why this role

Per diem W-2 role requiring a current California RN license, BLS, nursing license insurance, IV/phlebotomy certification, and at least one year of visiting home health RN experience. Availability requested for two to three days weekly (or ten productivity units), one weekend monthly, and two holidays yearly.
