What you'll do

• Assess patients’ psychosocial needs

• counsel and support patients and caregivers
• help navigate community resources, financial concerns, and care planning
• coordinate referrals and liaise with community agencies
• collaborate with nurses, physicians, and interdisciplinary teams
• participate in case conferences and discharge planning
• maintain timely EMR documentation.
