Home Health Coordinator

What you'll do

• Evaluate prospective clients alongside a nurse and complete service enrollment paperwork
• visit associates and residents in the field
• communicate resident needs to the home health team
• provide performance feedback and coaching
• conduct performance evaluations
• liaise among clients, direct care associates, and scheduling
• coordinate with healthcare providers and families
• maintain policy and regulatory compliance
• visit clients according to regulatory timing
• maintain records
• participate in the on-call rotation
• assist with payroll, schedules, and billing.
