Infusion Nurse – Home Infusion

The Infusion Nurse – Home Infusion is a per diem, onsite role serving De Soto, KS, and the immediate surrounding area. Compensation is not specified. The nurse supports Soleo Health’s home and alternate-site infusion services, which aim to simplify complex care. This role provides primary nursing care for patients receiving infusion therapy, including assessment, care planning, evaluation, and education. It helps patients receive prescribed therapy safely at home by coordinating with the care team, assessing the care environment, and documenting and following up on each encounter.

What you'll do

• Provide direct care and coordinate communications with pharmacists, prescribers, dietitians, care partners, and community resources
• educate patients and care partners on safe home infusion
• assess the care environment
• develop and evaluate plans of care with the team
• manage scheduling and coordinate care through approved platforms
• participate in the nurse on-call rotation
• document encounters and submit required treatment plans and clinical documents
• maintain knowledge of therapies, protocols, and clinical standards.
