What you'll do

• Identify care coordination needs with families
• coordinate family-based services in homes, schools, and communities
• support care planning and safety needs
• build relationships with children, families, and child and family teams
• develop community partnerships
• maintain documentation
• participate in program evaluation, planning meetings, and required training.
