Elder Care Navigator

What you'll do

• Coordinate care planning and facility placement or transfers
• support families in advocating with facilities and medical teams
• research community resources and facilities
• attend facility interdisciplinary team meetings as needed and document decisions
• assess functional levels and develop care recommendations with the legal team
• represent the firm to facilities and healthcare contacts
• maintain client confidentiality.
