What you'll do

• Plan social services programs with consideration for adult and geriatric developmental factors
• plan programs for residents with therapeutic, neurological, dementia, dialysis, IV therapy, and infectious disease needs as applicable
• follow up on admission inquiries and assess prospective residents for facility placement suitability.

Why this role

A bachelor’s degree in social work or a related field is preferred; at least one year of social work experience is required. Large long-term-care social work director experience is preferred.
