Community Outreach Specialist – Home Care and Home Health

The Community Outreach Specialist – Home Care and Home Health role is based onsite in North Kansas City, MO, with daily travel required. The base salary is $65,000–$80,000 per year. AbriCare provides personal care services intended to help seniors and adults with disabilities live safely and independently at home. The specialist grows branch census by developing referral relationships, converting referrals into starts of care, and increasing billable hours across Medicaid and private-pay services. The role combines community outreach, territory planning, and data-informed follow-up while maintaining compliance with applicable healthcare and Medicaid requirements.

What you'll do

• Analyze the market and develop a territory plan
• build referral partnerships with hospitals, facilities, physician offices, aging agencies, social-service agencies, and community organizations
• guide partners and families through the Missouri Medicaid DSDS process
• work with the branch to shorten time to start of care
• represent AbriCare at senior events, health fairs, and partner offices
• record activity in the CRM
• follow applicable anti-kickback, Stark Law, HIPAA, and Medicaid freedom-of-choice requirements.
