What you'll do

• Complete psychosocial assessments for hospice patients and families
• identify emotional, social, financial, and practical needs
• provide emotional support, counseling, and guidance
• assist with community resources, advance care planning, and end-of-life concerns
• support families experiencing anticipatory grief, loss, and transitions
• collaborate with nurses, chaplains, physicians, and other team members
• participate in interdisciplinary meetings as needed
• advocate for patient needs and dignity
• assist with crisis intervention when appropriate
• document in the EMR.
