What you'll do
- Develop and implement individualized care plans in accordance with physician orders
- assess patient needs and collaborate with the interdisciplinary team
- update plans as conditions change
- educate patients and families
- complete OASIS documents and visit notes
- communicate and document changes in patient status
- connect families with social work and community resources
- supervise LPNs and HHAs
- participate in patient and team conferences
- provide home-based care
- schedule visits and manage caseload
- precept newly hired nursing staff.
