What you'll do

• Participate in on-call duties per policy
• support the Quality Management program
• arrange patient equipment, supplies, and services
• review care, service utilization, skilled need, homebound status, documentation, communication, and discharge planning
• complete initial OASIS assessments
• establish nursing diagnoses and care plans with patient and family input
• provide prescribed nursing procedures, medications, and treatments
• supervise LPNs and home health aides per regulations
• reassess patients after condition changes or hospital discharge
• prepare clinical notes and update physicians at least every 60 days
• communicate patient needs and obtain orders
• collaborate with team members and the area team leader
• educate patients and caregivers.
