Case Manager, Substance Use Recovery

The Case Manager, Substance Use Recovery role is onsite in Chicago, IL, with compensation of $17.05–$23 per hour. The case manager provides individualized, goal-oriented support to help people served improve self-sufficiency and access recovery resources. Through assessment, service planning, referrals, advocacy, coordination, and monitoring, the role contributes to treatment and transitions to recovery or permanent housing.

What you'll do

• Conduct outreach and support participation
• assist people served with their goals
• connect them with community resources and social supports
• provide transportation to appointments
• provide and document case management and referral services
• coordinate transitions with licensed discharge agencies
• develop and present service plans in treatment plan reviews
• support care plans and aftercare
• identify resources and attend network meetings
• advocate and coordinate with family and community resources
• attend patient staffing or interventions
• seek supervision for clinical or programmatic decisions.
