Young Adult Case Manager

Young Adult Case Manager is an onsite role in Derry Village, NH, with stated pay of $26 per hour. Center for Life Management provides mental health services for children and families, including individuals ages 15–26 experiencing mental illness, serious emotional difficulties, or first-episode psychosis. The case manager coordinates care, resources, and community-based support to help clients improve symptoms, functioning, and wellness. The role works with clients, families, treatment colleagues, and external providers to carry out individualized treatment goals and maintain continuity of support.

What you'll do

• Assess clients and develop care plans
• assist with medical coverage applications
• coordinate outreach, advocacy, resources, and services
• provide counseling and functional support in home, school, or community settings
• provide therapeutic behavioral services and family training
• provide crisis intervention
• collaborate with treatment teams and external providers
• document services in accordance with agency and regulatory standards.
