MDS Director (RN/LPN)

The MDS Director (RN/LPN) role is onsite in Melbourne, FL, on a full-time Tuesday–Friday schedule with 10-hour shifts from 6:00 AM to 4:30 PM. What you'll do

• Oversee accurate, complete, and timely MDS assessments and documentation
• ensure compliance with state and federal regulations, Medicare, and PDPM requirements
• conduct resident assessments and interviews
• develop and monitor individualized care plans
• lead interdisciplinary care plan meetings
• educate nursing staff
• manage MDS and care plan schedules
• monitor documentation processes
• and partner with facility leadership to improve clinical outcomes and documentation.
