Hospice Chaplain

What you'll do

• Provide or coordinate spiritual care for patients, family members, and caregivers
• conduct spiritual assessments
• assist with bereavement follow-up in collaboration with the bereavement coordinator
• participate in care conferences, hospice admissions, and interdisciplinary group meetings
• support hospice team members
• maintain relationships with local spiritual leaders
• participate in quality improvement programs and services
• participate in the designated weekend on-call rotation.

Why this role
