What you'll do

• Direct and oversee social work services for skilled nursing residents
• complete psychosocial assessments and individualized care plans
• provide counseling, emotional support, crisis intervention, and advocacy
• participate in care plan meetings and interdisciplinary conferences
• assist with adjustment to skilled nursing and changes in health or function
• coordinate discharge planning and transitions between levels of care
• address concerns and grievances
• maintain required documentation
• support resident rights and person-centered care
• guide social services staff
• participate in quality improvement and regulatory surveys as appropriate.
