What you'll do

• Coordinate nursing assessments for prospective and newly admitted residents
• review hospital records, diagnoses, physician orders, medications, and clinical documentation
• collaborate with hospital case managers, physicians, facility leadership, and interdisciplinary team members
• complete admission documentation, medication reconciliation, and required assessments
• identify clinical needs and risks
• communicate with residents and families
• help ensure equipment, medications, treatments, and care resources are in place
• support referral-partner communication and comply with facility policies and regulatory requirements.
