What you'll do

• Conduct resident nutrition assessments
• develop, implement, and update individualized nutrition care plans
• provide nutrition counseling and education to residents, families, and staff
• collaborate with nursing, therapy, and dietary teams
• document assessments and progress according to regulatory standards
• support dietary compliance and food service quality, safety, and resident satisfaction. Full-time coverage may include two additional facilities as needed
• part-time coverage is 24 hours per week.
