Hospice Clinical Liaison (Registered Nurse)

What you'll do

• Build referral relationships with hospitals, skilled nursing facilities, assisted living communities, and physician practices
• educate physicians, discharge planners, and case managers about hospice eligibility and the Medicare hospice benefit
• review referrals and obtain clinical records
• coordinate complex cases with the Clinical Director
• discuss goals of care with patients and families
• hand off admitted patients to the RN Case Manager and interdisciplinary team
• track referral activity and document in the EMR.
