After-Hours Hospice Nurse (On-Call)

What you'll do

• Respond to after-hours patient and family calls with triage and guidance, and make in-person visits as needed
• conduct skilled nursing assessments and interventions in home or facility settings
• manage pain and symptom control according to care plans
• communicate patient status changes to the on-call physician and interdisciplinary team
• provide emotional support and education
• document clinical encounters
• coordinate medication changes, equipment, and emergency support.
