Afterhours RN Care Manager (7 on/7 off)

Assess and coordinate patient and family needs during after-hours shifts, maintain primary clinical oversight, conduct routine visits, admissions, death visits, and urgent after-hours calls. Participate in the Interdisciplinary Group (IDG) process, develop and revise the Hospice Plan of Care, educate patients and families, and maintain accurate clinical documentation.
