What you'll do
- Conduct and submit MDS assessments within required timelines
- establish assessment reference dates and manage scheduling
- develop and update individualized care plans
- coordinate with nursing, therapy, dietary, and social services
- monitor condition changes
- maintain documentation
- support Medicare Part A and PDPM documentation and reimbursement
- participate in QAPI initiatives
- educate nursing staff
- and assist with survey preparation.
