Home Health Field Registered Nurse (RN)

The Home Health Field Registered Nurse (RN) role is onsite in Tucson, AZ, providing care in patients’ homes. Compensation is not specified. The nurse supports Choice Health At Home’s home health services and works with patients, families, and interdisciplinary care teams. The RN assesses patient needs, develops and documents care plans, delivers and evaluates nursing interventions, and educates patients and families. The role promotes continuity of care, recovery or chronic disease management, and patient independence while communicating changes and outcomes to physicians and other care professionals.

What you'll do

• Conduct initial and ongoing comprehensive assessments of physical, functional, psychosocial, and environmental needs
• apply admission and recertification criteria
• develop, implement, document, and revise plans of care
• educate patients and families on self-care, nutrition, and dietary needs
• determine service scope and frequency based on acuity and needs
• assess caregiver ability to meet patient needs
• initiate preventative and rehabilitative nursing procedures
• apply infection-control and standard precautions
• provide clinical direction to Home Health Aides and LPNs/LVNs
• prepare clinical and progress notes
• inform physicians and other personnel of changes and outcomes
• monitor cases for third-party payer compliance
• communicate with patients, families, team members, and healthcare professionals.
